Registration Form: If you wish to register your school in the New York state Science Olympiad you may download this registration form for your use.

To use this form, please follow the following instructions:

  1. Download the form in color.
  2. Send this original form (with color) and a check for $205.00 to the address on the bottom of the form.
  3. Please note we do not accept purchase orders.

Form Packet: This file contains the Science Olympiad Code of Conduct, the Code of Conduct Signature form, the Science Olympiad policies on Cheating, Constructed Devices, Interference, Vandalism, and Calculators, and the Regional Final Registration form.

Talent Release Form: This file contains the form that each student MUST sign to allow us to use team photos, videos, and interviews in press releases and on the website. Division B Form, Division C Form.

Medical Form: This is the medical form that MUST be completed by the parents of every student who will be competing in the State competition (same form for both B and C divisions).
